How to file a claim
1. For insurance policy #NW0077, please contact Co-ordinated Benefit Plans at 866-223-4772 or e-mail your information to [email protected].
2. They will give you a claim number and the paperwork to fill out.
3. Complete the paperwork and send them the required documentation in a timely manner.
4. If you haven’t heard back from them within 15 days from the date you submitted the paperwork, please contact them by phone or email.
While this is the specific process for claims with Nationwide, there is a similar process with every insurance company. You need that company’s specific forms and instructions but receipts and other documentation are always necessary.
